Fontana Grande replaced an earlier fountain called Fontem Sepalis, documented in 1192. The name 'Fontana Grande' officially appeared in a list of fountains starting in 1565. The fountain's design features a Greek cross layout for the lower basin and a central pillar supporting two stacked cups topped by a finial. These elements are the result of refurbishments over the centuries, including those in 1279 and 1424. The four lion heads, symbols of Viterbo, were added in 1424. The pyramidal finial structures were added in 1827 by architect Domenico Lucchi. The fountain was originally fed by a Roman aqueduct that entered the city near the church of San Sisto.
