Walk along the main road (Havlíčkova) for about 1.5 kilometers until you reach the intersection with Palackého street. From there, continue walking straight onto Žižkova street, and follow it until you reach Šultysova street. The Plague Column will be on your left side at the address Šultysova, 284 01 Kutná Hora 1.
