From Sapporo Station, take the Namboku Line (Green Line) towards Makomanai. Ride for 4 stops and get off at Nakajima Koen Station. Exit the station and head towards the east exit. Once you exit, you will see Nakajima Park directly in front of you across the street.

If you are near Sapporo Station, you can take the bus from the Sapporo Station Bus Terminal. Look for buses heading towards Nakajima Park or Chuo Ward.
