Follow Highway 15 southbound, and keep an eye out for signs directing you towards Sassafras. After approximately 15 miles, you'll reach the exit for Smithboro Rd. Take this exit and continue on Smithboro Rd. for about 2 miles. The entrance to Carr Creek State Park will be on your left. The address is 2086 Smithboro Rd., Sassafras, KY 41759.
