Once you are in Mdina, head towards the Mdina Gate, which is the main entrance to the city. Exit Mdina through the gate and continue down the road towards the main bus station, known as the 'Bugibba Bus Station'. From there, look for the bus stop that has services to Cirkewwa. Take bus number 41 or 42, which will take you to the ferry terminal at Cirkewwa. The ferry will take you directly to Comino.
