Walking
If you are near Utrecht Central Station, exit the station and head towards the Vredenburg square. From there, walk straight towards the Vredenburg Theater. Continue straight to the Stadsschouwburg. Once you reach the theater, take a left onto the Voorstraat. Continue walking until you reach the intersection with the Vismarkt. Turn right onto the Vismarkt and continue straight until you reach the Dom Tower. From the Dom Tower, head towards the Oudegracht canal and take a left at the canal. Follow the canal until you reach the intersection with the Aisle Street (Aisleweg). Turn right onto Aisle Street and continue until you reach the intersection with the Croeselaan. Continue onto Croeselaan which leads you directly to the bus stop where you can catch bus 120 or 121 towards Amsterdam. Get off at Amsterdam, Rembrandtplein stop, and from there it's a short walk to Rembrandt Monument at Rembrandtplein 28.
Public Transport
From Utrecht Central Station, buy a ticket to Amsterdam. Take a direct train (Intercity) towards Amsterdam Centraal, which takes about 30 minutes. Once you arrive at Amsterdam Centraal, exit the station and walk towards the tram stop located just outside the main entrance. Take tram number 4 or 14 towards the direction of 'Rembrandtplein'. After about 10 minutes, get off at the 'Rembrandtplein' stop. From the tram stop, walk a short distance towards Rembrandtplein 28. The Rembrandt Monument is located in the square itself.
