From the Island in the Sky Visitor Center, head southeast on the main road (UT-313) for about 9 miles until you reach the intersection with UT-191. Turn left (south) onto UT-191 and continue for approximately 22 miles. Then, turn left onto UT-46 towards Monticello. Follow UT-46 for about 17 miles. After passing through Monticello, continue on UT-46, which will turn into a dirt road (Needles Overlook Road). Follow this road for about 6 miles until you reach the Needles Overlook parking area. There are no fees for parking at the overlook.
