From Medellín, go to the Terminal de Transportes del Norte, the main bus terminal. Purchase a ticket for a bus to Andes; the journey takes about 4 hours. Buses typically run every hour, and the cost is around 30,000 COP. Once you arrive in Andes, you can walk to the town center or take a taxi if you have heavy luggage. Make sure to check the schedule for your return trip, as buses may have limited evening hours.
