Walking
From the center of Mindo, head towards the main road (Via a Mindo). Walk towards the northwest direction for about 15-20 minutes until you reach the bus stop for Quito. Take a bus heading to Quito, which will take approximately 2-3 hours. Once you arrive in Quito, ask the bus driver to let you off at the closest stop to the historic center. From there, walk towards Calle Guayaquil, which is about a 10-minute walk. The Iglesia de Santo Domingo will be located at C. Guayaquil S1-76, and should be clearly visible along the street.
Public Bus
From Mindo, walk to the main bus terminal where buses to Quito depart. Buses leave frequently, so you shouldn't have to wait long. Once on the bus, enjoy the scenic ride, which will take around 2-3 hours. Upon arriving in Quito, disembark at the terminal. From there, you can take a taxi or walk to the historic center; it's about a 10-15 minute walk. Head towards Calle Guayaquil, where Iglesia de Santo Domingo is located at C. Guayaquil S1-76.
