If arriving at Bremen Hauptbahnhof (main train station), take any tram (lines 2, 3, 4, 6, or 8) to the Domsheide stop. From Domsheide, walk a short distance (about 1 minute) towards the Bremen Cathedral; the fountain is in front of it. A single tram ticket costs €2.85.
