Get off at the 'Yad Vashem' station, which is the closest stop to Yad Vashem.

Make your way to the Holon - Wolfson train station. Board a train heading towards Tel Aviv. Change trains at the Tel Aviv Savidor Center station to a train heading to Jerusalem.
