If you are starting from the Modi'in train station, exit the station and head east on HaYarden Street. Continue straight until you reach the intersection with Lev Ha-Ir Street. Turn left onto Lev Ha-Ir Street and walk for about 1.5 kilometers. You will see signs for האגם של יעקב as you approach the area.
