The train ride takes approximately 50 minutes. After arriving at Milano Centrale station, exit the station and follow the signs to the Metro. Take the Metro Line 3 (the yellow line) towards Duomo. After 10 stops, you will arrive at the Duomo station. Exit the station, and you will be just a short walk away from the Duomo di Milano. Follow the signs for P.za del Duomo, and you will find the cathedral in the main square.
