If you are near the Duomo, take the M1 (Red Line) metro from Duomo station towards Sesto 1 Maggio FS. Get off at the 5th stop, which is 'Cadorna'. Transfer to the M2 (Green Line) metro towards Abbiategrasso. After 3 stops, get off at 'P.ta Genova FS' station. Exit the station and walk straight along Via Genova until you reach Corso di Porta Ticinese. Turn left and you will find Porta Ticinese Medievale at number 51b.
