From the Galleria Vittorio Emanuele II, simply exit the arcade into Piazza del Duomo. The Duomo will be directly in front of you. From Teatro alla Scala, walk straight along Via Santa Margherita, then turn right onto Piazza del Duomo. The walk is approximately 5-10 minutes. No costs involved.
