Walking
From the Mdina Gate, exit the fortified city and head straight down Triq Villegaignon. Continue walking until you reach the main road, Triq Naxxar. Turn left and follow this road for about 1.5 kilometers until you reach the intersection with Triq il-Labour. Here, you will turn right onto Triq il-Labour and continue straight. After about 500 meters, you will see the Mosta Rotunda on your left at Rotunda Square. The church is easily recognizable due to its large dome.

From Mdina, walk to the nearest bus stop located at the Mdina Gate. Catch bus number 81 or 82 towards Valletta and get off at the Mosta stop. From there, walk towards Rotunda Square, where you will find the Mosta Rotunda. The church is just a short walk from the bus stop, and it should take you about 5-10 minutes.
