If you are near the main train station (Hlavní nádraží), take tram number 1 or 2 towards 'Bystrc, Kamechy' or 'Slatina'. Get off at the 'Zelný trh' stop. From there, walk straight along the street towards the 'Jakubské náměstí' (Jakub Square). The church will be on your right at Jakubské nám. 11.
