You'll see signs for Dämmchen. The island is a short walk from there, with access via small bridges. The walk from Domplatz takes approximately 5-10 minutes. Keep an eye out for the 'Zu den kleinen Füchsen und roten Hirsch' house on Hütergasse, which marks the entrance to Dämmchen.
