If you're starting at Leipzig Hauptbahnhof (the main train station), head to the tram station located just outside the main entrance. Take tram line 9 towards 'Leipzig, Lößnig' and ride for 5 stops (approximately 10 minutes). Get off at 'Johannisplatz'. From the tram stop, walk back towards the direction the tram came from for about 2 minutes. You will find Kreuz an der Stelle der Ehemaligen Johanniskirche located at Johannispl. 5-11.

If you are near the city center, you can walk to Kreuz an der Stelle der Ehemaligen Johanniskirche. From the Marktplatz, head southeast towards the Nikolaikirche. Continue straight until you reach the intersection with Karl-Liebknecht-Straße. Turn left and walk until you reach Johannisplatz, where you will find the church at Johannispl. 5-11.
