If you are near the Palazzo Ducale, exit the palace and walk south on Via Garibaldi. Continue walking straight, and it will lead you to Piazza Grande. It’s approximately a 10-minute walk from the palace.

If you're at the Modena bus station, take bus line 6 or 7 towards the city center. Get off at the stop 'Piazza Matteotti.' From there, walk a short distance down Via Emilia Centro, and you will arrive at Piazza Grande in about 5 minutes.
