Walking
From the center of Butha-Buthe, head towards the main road (Mokema Road). Walk southeast on Mokema Road until you reach the junction with the R70 road. Continue straight on Mokema Road for about 1.5 kilometers until you see a sign for Mokema. After the sign, you will begin to see the landscape change as you approach the outskirts of the town. Keep walking for another 2 kilometers until you reach the entrance to the Underground Cave, which is located at GM2H+5Q, Mokema. Look for a small path leading off the main road, which will guide you to the cave entrance.

From your location in Butha-Buthe, find a local taxi or shared minibus taxi (known as 'kombis') heading towards Mokema. You can ask locals for the nearest taxi rank. Once you board a taxi, inform the driver that you are going to Mokema. The ride should take about 10-15 minutes, depending on traffic. Ask the driver to drop you off near the Underground Cave, or at the nearest landmark, such as the sign for Mokema. From there, you may need to walk approximately 500 meters to reach the cave entrance. Expect to pay around 10-20 LSL for the taxi ride.

To reach the Underground Cave in Lesotho by car, you will need to drive towards the town of Maseru. From Maseru, take the A2 road towards the town of Roma. Continue on the A2 until you reach the town of Morija. From Morija, follow the signs towards the Ha Kome caves. The Underground Cave is located near the Ha Kome caves, so you can ask for directions once you reach that area.
