From the main square in Cabo Rojo, head northeast on Calle Dr. Pedro S. Cordero towards Avenida José de Diego. Turn left onto Avenida José de Diego and continue straight until you reach the intersection with Calle Cañón. Turn right onto Calle Cañón, which will lead you towards the coastal area. Continue walking until you reach the entrance of the Cabo Rojo National Wildlife Refuge. Once inside, follow the signs towards Cueva Del Pirata Cofresí. The path is well marked, and you may need to walk approximately 1 mile until you reach the cave.
