If you are using public transportation, take a bus from San Juan to Río Grande. Buses usually depart from the San Juan bus terminal. Once in Río Grande, you can use a taxi or rideshare service to reach PR-191. Inform the driver that you are going to Juan Diego Falls. It's approximately a 10-minute drive from the town to the falls.
