From the center of Bethlehem, head southwest towards the Manger Square. Walk past the Church of the Nativity and continue straight until you reach the main road. Turn left onto the main road (Bethlehem Road) and walk for approximately 15 minutes until you reach the junction with the road to Beit Jala. Continue straight until you see signs for Tel Azekah. Follow the signs for another 10 minutes, and you will arrive at Tel Azekah.
