You can take the East West Line to Tanah Merah, and then transfer to the Downtown Line to reach Marina Bay. From Marina Bay, you can walk to Marina South Pier. The MRT fare will cost you around SGD 2-3. At Marina South Pier, purchase a ferry ticket to Lazarus Island. The ferry ride typically costs around SGD 18-25 and takes about 30 minutes.
