If you're in the city center, make your way to the Vrijthof square. From there, head southeast towards the St. Servaasbrug (Saint Servatius Bridge). Cross the bridge and continue straight on the street called 'Sint Bernardusstraat'. Once you reach the end of the street, turn left onto 'Onze Lieve Vrouweplein'. The Basilica of Our Lady will be on your right at number 7. The walk should take you about 10 minutes.
