Take the exit onto OR-58 E and continue for approximately 30 miles. You will then merge onto US-97 N toward Bend. After about 20 miles, take exit 153 toward Cottonwood Rd. Follow Cottonwood Rd for approximately 5 miles, and you will find the entrance to Lava River Cave on your right. There may be a parking fee of around $5, so be prepared with cash or a card.
