If you are in the Old City of Jerusalem, head towards the Jaffa Gate. From the Jaffa Gate, walk along the main street (David Street) until you reach the intersection with King David Street. Turn left and continue walking until you reach the intersection with Agron Street. Turn right onto Agron Street and continue straight until you reach the intersection with Hillel Street. Turn left onto Hillel Street, and continue until you reach the Mount of Olives area. From there, follow the signs towards the Tomb of Torat Chesed Rabbi Zalman Fradkin, which is located in the vicinity of the Mount of Olives. The tomb is marked and should be visible from the pathways.

From the central bus station in Jerusalem, take bus number 75 or 77 towards the Mount of Olives. The bus will take you directly to the area near the Tomb of Torat Chesed Rabbi Zalman Fradkin. Once you disembark, follow the signs that lead to the tomb. It should be a short walk from the bus stop to the actual site.
