Take tram number 1, 2, or 12 from any central tram stop (e.g., Centraal Station, Dam Square) towards the direction of Vondelpark. Get off at the stop 'Vondelpark'. From there, walk into the park and follow the paths until you reach Vondelpaviljoen, located at Vondelpark 3.
