Walking
From St. Peter's Square, exit the square through the colonnade and head towards the main street, Via della Conciliazione. Walk straight along this road until you reach the Tiber River. Once you arrive at the river, turn left and follow the riverbank for about 1 kilometer. After walking alongside the river, you will come to the Ponte Sisto (Sisto Bridge). Cross the bridge, and as you come down the steps on the other side, you will find yourself in the Trastevere area. Continue straight until you reach Piazza di Santa Maria in Trastevere, where the Basilica is located.

Walking
Alternatively, from St. Peter's Square, walk towards the entrance of the Vatican Museums. Just before you reach the entrance, turn right onto Via di Porta Angelica. Continue straight until you reach the intersection with Via di San Giovanni in Laterano. Here, turn left and continue until you reach the Tiber River. Turn right and follow the river until you reach the Ponte Sisto. Cross the bridge and follow the previous instructions to Piazza di Santa Maria in Trastevere.

Public Transport
From St. Peter's Square, walk to the nearest bus stop on Via della Conciliazione. Take bus number 23 towards 'Piazza Venezia'. Get off at the stop 'Piazza San Giovanni in Laterano' and transfer to tram line 8. Board the tram towards 'Arenula-Cairoli' and get off at the stop 'Trastevere'. From there, it is a short walk to Piazza di Santa Maria in Trastevere where the Basilica is located.
