If you're starting from the Rialto Bridge, walk south along the Calle dei Fabbri until you reach the Campo San Bartolomeo. From here, take the Calle del Lovo to your left. Continue straight, and you will reach the Campiello San Giovanni. The Church of San Giovanni Elemosinario will be on your right at Ruga Vecchia S. Giovanni.
