Walking
If you are in central Rancagua, start at Plaza de Los Heroes, which is a central point. From the Plaza, walk south down Avenida O'Higgins for about 10 minutes until you reach the intersection with Calle Maipú. Turn right onto Calle Maipú. Continue walking straight for another 5 minutes. You will see the Memorial 16 Estrellas on your left at the end of Calle Maipú, located in a small park area.

From Plaza de Los Heroes, walk to the nearest bus stop on Avenida O'Higgins. Take a bus heading to the 'Cerro San Ramón' or similar route that passes near Parque O'Higgins. After approximately 10-15 minutes, get off at the stop near Calle Maipú. From there, walk straight down Calle Maipú for about 5 minutes until you reach Memorial 16 Estrellas, which will be on your left.
