If you are in the center of Amsterdam, head towards the Dam Square. From Dam Square, walk towards the Nieuwezijds Voorburgwal street. Continue walking straight until you reach the intersection with the Muntplein. Turn left onto the Muntplein and continue straight until you reach the Amstel River. Cross the bridge over the Amstel and follow the path until you see the Westermarkt street on your left. Turn left onto Westermarkt. The Homomonument is located on your right, just a short walk down Westermarkt.
