From St. Peter's Square, exit the square towards the right side of the Basilica, heading towards Via della Conciliazione. Walk straight along this wide avenue until you reach the first intersection. Turn left onto Via dei Corridori and continue walking straight. After a few minutes, you will reach a small square. Santa Maria in Cosmedin is located at Piazza della Bocca della Verità, 18, just a short walk from this square.

Another option is to head out of St. Peter's Square and go to the right side of the Basilica, following the crowd towards the Ponte Sant'Angelo. Cross the bridge and continue straight until you reach the intersection with Via della Conciliazione. Turn left and continue walking until you reach the Piazza della Bocca della Verità. The church is right at that location, number 18.

Once you reach the river's edge, turn left and stroll along the riverbank until you reach the Ponte Sant'Angelo.
