Walking
To reach Santa Maria in Cosmedin from the Vatican Gardens, start by exiting the Vatican Gardens. Head towards the entrance gate of the Vatican City, which will bring you to the street Via di Porta Angelica. Once you are out of the Vatican, turn left onto Via di Porta Angelica and follow it until you reach the intersection with Via della Conciliazione. Continue straight on Via della Conciliazione, passing by the Castel Sant'Angelo on your right, until you reach the Tiber River. Cross the river via the Ponte Sant'Angelo. After crossing, turn right onto Lungotevere in Augusta, and then take the first left onto Via dei Giuochi Istorici. Follow this street straight until you reach the intersection with Via di Santa Maria in Cosmedin. Turn right, and Santa Maria in Cosmedin will be on your left at Piazza della Bocca della Verità, 18.

From the Vatican Gardens, exit towards the main entrance of Vatican City and head towards the nearest bus stop. Board bus number 64 towards Termini. After about 10 minutes, get off at the stop called 'Piazza Venezia'. From there, walk towards Via dei Fori Imperiali, and then take a left onto Via della Consolazione. Continue walking straight until you reach Via di Santa Maria in Cosmedin. Turn left onto this street, and Santa Maria in Cosmedin will be on your left at Piazza della Bocca della Verità, 18.
