Walking
From St. Peter's Square, head towards the main entrance of St. Peter's Basilica. Once you exit, walk straight ahead towards the large road (Piazza San Lorenzo in Lucina). Continue walking straight until you reach the first major intersection. Turn right onto Via della Conciliazione and walk until you reach the Tiber River. Cross the Ponte Sant'Angelo bridge. After crossing, continue straight until you reach the intersection with Via dei Coronari. Turn left and continue walking straight until you reach Piazza della Bocca della Verità. The Mouth of Truth is located at the entrance of the Basilica di Santa Maria in Cosmedin, which is at the far end of the square.

Public Transport
From St. Peter's Square, walk to the nearest bus stop located at Piazza San Pietro. Take bus number 23 or 280 towards the direction of Piazza Venezia. Get off at the stop called 'Piazza Venezia'. From there, walk down Via del Teatro di Marcello until you reach the intersection with Via della Bocca della Verità. Turn right onto Via della Bocca della Verità, and you will find the Mouth of Truth at the Basilica di Santa Maria in Cosmedin.
