If you are starting from the Pile Gate, which is one of the main entrances to the Old Town, head straight down the Stradun, the main street in Dubrovnik. Continue walking for about 400 meters until you reach the Luža Square. The Church of St. Blaise is located on this square, at Luža ul. 2. It is a prominent building with a distinctive facade, and you can easily spot it.
