Continue walking until you reach the end of the canal where it meets the Grand Canal. At this point, turn left and follow the Grand Canal until you reach the Ponte dei Tolentini bridge. Cross the bridge and you will find yourself on Fondamenta dei Tolentini. This walk should take you about 15-20 minutes.

Vaporetto (Water Bus)
If you prefer a quicker route, you can take the Vaporetto. Head to the nearest Vaporetto stop, which is likely 'San Stae' or 'Riva di Biasio' in Santa Croce. Purchase a ticket from the ticket booth or a machine (a single ride costs approximately €7.50). Board the Vaporetto heading towards 'Piazza San Marco' or 'Zattere'. Get off at the 'Tolentini' stop. From there, it's just a short walk to Fondamenta dei Tolentini. This method will take around 10 minutes on the Vaporetto, plus the walking time.
