If you're driving from the East Bay, take the I-80 W towards San Francisco. You will cross the Bay Bridge, which has a toll of approximately $7 (cash or FasTrak). Once in San Francisco, continue on I-80 W and take the exit for Fremont St. Follow Fremont St, which will turn into The Embarcadero. Continue on The Embarcadero until you reach the intersection with Bay St, then turn right onto Bay St. Follow Bay St until you reach Stockton St and turn left. The Hearts of San Francisco Art Sculpture is located at the intersection of Stockton St and Geary St in Union Square.
