From the Sistine Chapel, exit the chapel and walk towards the Vatican Museums entrance. Continue walking towards the main entrance of the Vatican City towards St. Peter's Square. Once at St. Peter's Square, exit the square through the main gate and head towards Via della Conciliazione. Follow Via della Conciliazione until you reach the Tiber River. Cross the river using the Ponte Sant'Angelo. After crossing, turn left and walk along the riverbank until you reach Ponte Fabricio, which is located on the Isola Tiberina.

Alternatively, from the Sistine Chapel, head towards the Vatican Museums exit and then make your way to the Ottaviano metro station. From there, take Line A (the red line) towards Anagnina. Get off at the Barberini stop. Exit the station and walk towards Via del Tritone. Continue straight until you reach the Tiber River, then turn left and walk along the river until you reach Ponte Fabricio.
